New Bariatric Patient Appointment Check List

In preparation for your first visit with your bariatric team, here is a list of tasks to help you prepare for a smooth successful visit.

  • Bring all of your current medications and supplements to every doctor visit.
  • Identify a support person to attend all appointments. We want to be able to accommodate your entire family but simply don’t have the space for more than one family member during our educational sessions.  (Only exception will be the initial psychology appointment, which the support person will not be able to attend.)
  • Have copies of any recent medical test or workup sent directly to you. Please make a copy of these for your records and bring us a copy to scan into your medical history. Please do NOT have them faxed, mailed, or emailed to us unless explicitly directed by our staff. Obtaining and bringing the below records may reduce the number of tests required:
    • Blood Test
    • Endoscopy and Colonoscopy reports
    • X-ray reports
    • EKG’s
    • Sleep Study Results
  • Recent Notes and tests from Cardiology, Pulmonology, Endocrinology, Hematology, Primary Care Physician, and Medical Weight Loss Provider.
  • If you have had cancer within the last ten years, clearance from your oncologist
  • Obtain copies of any actual operative report and bring, if possible, from any surgeries on your abdomen.
  • Ask if anyone in your immediate family has had problems with bleeding during surgery or blood clots, including blood clots in the legs (known as DVT or Deep Venous Thromboembolism) or lungs (known as PE or Pulmonary Embolism).  If they have had any of these occur, ask if any workup was done to determine the cause.  If a reason were found, any documentation about this would be helpful.
  • Read as much as possible and educate yourself about the bariatric options and the behaviors you must master to make it successful.
